Addressing the Opioid Crisis: Translating PROSPER Prevention Science Into Rural Community Practice

The 2016 Surgeon General Report on addictions in America indicated it was necessary to invest in dissemination of proven prevention, particularly through community-based prevention delivery systems. The current opioid crisis underscores how such investment in proven prevention delivery systems is critically important. This presentation summarizes PROSPER—a cost effective delivery system for evidence-based prevention programs designed for and rigorously tested in state Extension Systems. It will cover five key topics. First, it will outline the rationale for prevention-focused delivery systems, especially in rural communities. Second, it will provide an overview of the PROSPER delivery system and programs on the PROSPER menu. Third, it will summarize wide-ranging results from a longitudinal, controlled study following participants through young adulthood, including long-term opioid misuse outcomes. Fourth, it will review PROSPER adaptations specifically focused on enhancing positive opioid misuse outcomes. Finally, it will address initial steps in state adoption of the PROSPER system.

Richard Spoth, Ph.D., is the F. Wendell Miller Senior Prevention Scientist and the Director of the Partnerships in Prevention Science Institute at Iowa State University. Over the past 25 years he has conducted extensive research on the dissemination of universal evidence-based programs through community-university partnerships. With this work, he has received the Prevention Science Award from the Society for Prevention Research for outstanding contributions to advancing the field of prevention science, as well as the Service to the Society for Prevention Research Award, the Translation Science Award, and the Presidential Award for lifetime scientific achievement.
