Exploring Barriers of Early Pediatric Cochlear Implantation
Cochlear Implant (CI) is surgical procedure to treat congenital or acquired bilateral severe to profound sensori-neural hearing loss with no or limited benefit from strong gain hearing aids. Early CI leads to many benefits for deaf children including increased language ability and an overall higher quality of life .Study explored the mean age of CI under Government-funded scheme in Kerala, India and analyzed the barriers of early pediatric CI. Mean age and barriers were analyzed for late implantation at various stages such as hearing screening, confirmation of hearing loss, hearing aid fitting, and age of surgery. The barriers were analyzed into 4 categories like system delay, medical delay, parent-related delay and other delays. The delay at each stages was identified by the questionnaire by team members. The mean age of hearing screening was 6.97±8.08 months, confirmation of hearing loss was 12.97±9.035 months, hearing aid fitting was at 18.69±9.829 months and age of cochlear implantation was 35.62 ± 10.383 months. There were 23 months of mean age (p=<0.001) elapsed between diagnosis of hearing loss and CI surgery. The most significant barrier was system delay at hearing screening stage (45%), medical delay (50%) at confirmation stage, system delay at hearing aid fitting (52.4 %) and other delay (45%) at CI surgery . The barrier reflects that the children undergoing cochlear implantation under the Government funded scheme faces delay at all stages . This study also confirms substantial number of children with hearing loss can be expected to receive their first cochlear implant well beyond their first year of life if we fathom out all barriers at each stage for better outcomes. Subsequently these barriers could be overcome with better government policies and programs, leading to early detection and intervention of hearing loss in children.
