Working with Stakeholders to Improve Care Coordination
HB2255 was passed in 2019 giving the Statewide Outreach Center (SOC) at Texas School for the Deaf access to the TEHDI system. This has allowed SOC to communicate with families and professionals to make sure identified children are receiving services as soon as possible.
This presentation will share details on how SOC partners with TEHDI as well as statewide and local EI professionals and other key stakeholders to ensure care coordination. We will provide strategies that have been implemented and proven effective in our state.
- Participants will be able to identify 1 or more strategies to assess whether it can be utilized in their current efforts
- Participants will be able to list critical stakeholder relationships for care coordination
- Participants will be able to extrapolate 1 or more components from the TEHDI/SOC collaboration to be applied in their state

Shelly works as the Intervention Care Coordinator for the Statewide Outreach Center at Texas School for the Deaf. In this position she partners with TEHDI and other stakeholders to reach out to families with newly identified infants as well as analyze data to improve the Loss to Follow up and Loss to Documentation (LTF/LTD) in Texas. She and her husband Jon live in Austin, Texas and have two sons who are Deaf.
