The Use of Antiseptics Rather than Antibiotics in Active Chronic Otitis Media (COM)

Background: The literature has established that in active chronic otitis media (COM) local antibiotics are better than oral and than ear toilet alone; and that quinolone +/- steroid eardrops are the current gold standard for effectiveness. Howeve, these antibiotic eardrops have some disadvantages, especially in the emerging world - not the least being price. Aim: To describe our research on the use of local antiseptic therapy in active COM. Method: We conducted 2 studies: a large-scale randomized controlled trial on the use of boric acid powder in active COM; and an in vitro study of several other potentially useful antiseptics. Results: We found boric acid powder, administered once-off according to our technique, was as effective as quinolone drops. And povidone iodine 5% solution shows in vitro potential as another antiseptic agent.
